๐ก PRESUMPTIVE SIGNS
(Subjective findings โ felt by the woman; not diagnostic)
๐ก Amenorrhea
๐น Amenorrhea refers to the absence of menstruation, often the first change noticed in early pregnancy.
๐น It occurs because hormonal changes suppress ovulation and the menstrual cycle.
๐น This sign is not specific to pregnancy and can result from stress, weight changes, endocrine disorders, or excessive exercise.
๐น Breastfeeding and irregular cycles further reduce its reliability.
๐น It is considered presumptive because it is reported by the woman and not directly observed by the examiner.
๐ก Fatigue, nausea, and vomiting
๐น These symptoms are common in early pregnancy due to increasing hCG and progesterone levels.
๐น Nausea and vomiting (โmorning sicknessโ) may occur at any time of day, especially in the first trimester.
๐น Fatigue results from increased metabolic demands and hormonal effects.
๐น These symptoms are nonspecific and can also occur with infections, GI illness, anemia, or stress.
๐น Because they are subjectively experienced, they are classified as presumptive signs.
๐ก Breast tenderness
๐น Breast tenderness occurs due to estrogen and progesterone stimulation of breast tissue.
๐น Women may report swelling, tingling, heaviness, or sensitivity early in pregnancy.
๐น These changes prepare the breasts for lactation.
๐น Similar symptoms can occur with premenstrual syndrome or hormonal therapy.
๐น Since the finding is felt by the woman, it remains a presumptive sign.
๐ PROBABLE SIGNS
(Objective findings โ observed by the examiner or tests; strongly suggest pregnancy)
๐ Chadwickโs sign
๐น Chadwickโs sign is a bluish-purple discoloration of the cervix, vagina, and vulva.
๐น It is caused by increased pelvic blood flow and vascular congestion during pregnancy.
๐น This sign typically appears around 6โ8 weeks of gestation.
๐น It is detected on pelvic examination by a clinician.
๐น It is probable because other conditions causing pelvic congestion can mimic it.
๐ Positive hCG pregnancy test
๐น Pregnancy tests detect human chorionic gonadotropin (hCG) in blood or urine.
๐น hCG is produced by trophoblastic tissue after implantation.
๐น False positives may occur with trophoblastic disease, certain tumors, or fertility medications.
๐น False negatives may occur if testing is done too early.
๐น Because hCG does not directly confirm a fetus, it is a probable sign, not positive.
๐ Fetal movement felt by experienced clinician
๐น During abdominal or pelvic examination, a clinician may feel movement within the uterus.
๐น This movement suggests the presence of a fetus suspended in amniotic fluid.
๐น It is more reliable than maternal quickening but still not definitive.
๐น Intestinal activity or abdominal muscle movement may mimic fetal motion.
๐น Therefore, it is classified as a probable sign.
๐ข POSITIVE SIGNS
(Diagnostic โ direct evidence of pregnancy)
๐ข Auscultation of fetal heart tones via Doppler
๐น Hearing fetal heart tones confirms the presence of a living fetus.
๐น Fetal heart tones are usually detected by Doppler at 10โ12 weeks gestation.
๐น The fetal heart rate is distinct from the maternal pulse.
๐น No other condition can produce fetal heart tones.
๐น This finding is a positive (diagnostic) sign of pregnancy.
๐ข Ultrasound verification of embryo or fetus
๐น Ultrasound provides direct visualization of the embryo or fetus.
๐น It can confirm location, viability, and gestational age.
๐น Cardiac activity may be visualized early in pregnancy.
๐น Ultrasound eliminates uncertainty caused by subjective symptoms or indirect signs.
๐น It is the most definitive confirmation of pregnancy, making it a positive sign.
